Medicare

I take original Medicare insurance with or without supplemental insurance and Medicare PPO. Co-pays, if any, are due at time of treatment. Medicare HMO plans are not accepted.

 

PPO

If you have a PPO – payment is due by cash/check/credit card at time of treatment. I will issue you a superbill which you can submit for reimbursement from your insurance provider (typically up to 80% is reimbursed after you have met your deductable).

 

You can use a service like Reimbursify on Apple/Android to file these claims on your behalf – $3 per claim ($2 per claim prepaid in blocks of 10) – first claim free

Other

Cash/Check/Credit Card payment is due at time of treatment.

 

Returned checks

Returned checks will be subject to a returned check fee of $25 and future personal checks will no longer be accepted.

 

First visits are about 1.5 hours long. Subsequent visits are about 1 hour long.

First visits typically cost from $250 to $360 depending how much is going on and how much is treated. Subsequent visits typically from $175 to $230.
