Insurance
Medicare
Dr Shupe accepts and bills original Medicare insurance with or without supplemental insurance and Medicare PPO. Medicare Advantage and Medicare HMO plans are not accepted.
Blue Shield PPO
Dr Shupe accepts Corporate/Government Blue Shield PPO – for health care reform Blue Shield PPO you must meet your out of network deductible before they will reimburse for visits.
Out of Network PPOs
When you use your PPO plan out-of-network – you pay for services received and Dr Shupe will issue a superbill which you can submit to your insurance company. If you have not yet met your out-of-network deductible for the year the visit fees are applied towards your deductible. Once you have met your out-of-network deductible for the year, then your insurance company will reimburse a percentage of the cost of your visit (Typically 50-80%).
Payment
Payment of co-pays, deductibles, and out of network services are due at time of treatment. I accept Cash, Check, and Credit Cards.
Returned checks will be subject to a returned check fee of $25 and future payment by check may be declined.
Visits
First visits are typically 60 – 90 minutes. Subsequent visits are typically 45-60 minutes.

Costs
Initial visits typically cost from $250 to $355 depending how much is going on and on the time available. Subsequent visits typically cost from $195 to $250.
